Residential Palliative Care is now offered in Illinois.

Available as a stand-alone service or paired with home health care with Residential Home Health and the Comfort Path program, Residential Palliative Care takes a focus on symptom management and quality of life to the next level. With consultations from an Advanced Practice Nurse (APN) who is advanced certified in hospice and palliative care (ACHPN), Residential Palliative Care provides a comprehensive assessment of the physical, psychosocial, and spiritual aspects of a patient’s illness experience and makes recommendations to address the individual needs of each patient and family.
