Residential Home Health for Patients

Referral Process

We are available to take referrals 24 hours a day, 7 days a week, including holidays and weekends. We service more than 30 Michigan counties.

If the patient is in the hospital:

  1. The patient or family member should tell the physician and the hospital discharge planner that they want a homecare referral.

  2. The patient or family member may request Residential Home Health if this is their agency of choice. Everyone has the right to choose the homecare service they want. If you know which agency you would like to provide your services, you must specify. Failure to specify a particular agency, such as Residential Home Health, will likely result in your physician or hospital discharge planner randomly selecting a service for you.

  3. Before the patient is discharged from the hospital the patient or family member must verify that a homecare order has been written and, if a specific agency was requested, that information is on the order.

  4. Residential Home Health will admit a patient within 24 hours of receipt of the referral, unless otherwise specified.
